To help us provide you with the best possible care, we ask that you:

  • Treat staff and healthcare providers with respect.
  • Provide complete and accurate health information, including updates to your condition, symptoms, or allergies.
  • Keep your contact details current, and provide at least one (ideally two) emergency contacts so we can reach you if needed.
  • Follow up on test results by booking an appointment if you haven’t received them.
  • Ask questions if you don’t understand your treatment or what’s expected of you.
  • Participate in and follow the treatment plan you’ve agreed to—or discuss alternatives if you choose not to.
  • Respect clinic guidelines, including no perfumes/cologne, no smoking, mask-wearing, and COVID screening.
  • Pay for any third-party services not covered by OHIP.
  • Keep scheduled appointments or give us notice if you need to cancel.

  • Receive care without discrimination of any kind.
  • Have your health information kept confidential, as required by law.
  • Receive clear, understandable information about your care and treatment.
  • Be involved in making decisions about your care.
  • Accept or refuse treatment, unless legally restricted.
  • Ask questions and voice concerns without affecting your care.
  • Request your medical records be transferred (a fee may apply).
  • Be informed of any fees not covered by OHIP.
